Harken has been informed that a customer received a TerrAdaptor Portable Anchor System without the three internal screws that secure the Half Plate to the Bolt Plate. This condition was identified during initial use. No injuries occurred. Harken is committed to ensuring the safety, reliability, and confidence of all users of our equipment. After learning about this issue, we completed a review of all relevant assembly procedures to help prevent recurrence.
Although we believe this to be an isolated incident, we request that all customers in possession of a TerrAdaptor Portable Anchor System with a Half Plate marked lot number 11624 perform an inspection at their earliest opportunity.
1. Confirm that your Half Plate is marked with lot number 11624.
2. Using a 3/8 hex wrench, remove the four large bolts securing the Half Plate Assembly to the Main Plate.
3. Inspect the reverse side of the Bolt Plate and verify that all three internal screws are properly installed.
If all screws are present:
- Reassemble the Half Plate Assembly to the Main Plate using the four large bolts. • Torque bolts to 38–42 lb-ft (51.5–57 Nm). • The TerrAdaptor may be placed back into service.
- Torque bolts to 38–42 lb-ft (51.5–57 Nm).
- The TerrAdaptor may be placed back into service.
If any of the internal screws are missing:
- Please remove the TerrAdaptor from service.
- Contact Harken Customer Service at 262-691-3320 or srsupport1@harken.com so we may provide support.
REMEDY:
If your inspection identifies missing internal screws, our Customer Service team will provide a prepaid shipping label and direct you to send the Half Plate Assembly to the nearest Harken facility. We will supply a replacement Half Plate Assembly promptly and at no cost to you.We appreciate your cooperation and apologize for any inconvenience this situation may cause.
